The last page of the claim form is the “initial medical certificate for an accident victim.” Your doctor must fill out all the details on the certificate and sign it. The form signed by the doctor should then be attached to the claim form and submitted to the National Insurance Institute.
A salaried employee must attach to the claim certification from his employer regarding his absence from work due to the accident, and the number of sick leave days available to him, along with certification of his salary on the appropriate form.
The claimant must cooperate with the National Insurance Institute in determining his entitlement by undergoing examinations and providing any information or document pertaining to the accident, as required by the NII.
