A claim or benefits should be submitted on the form Application to establish a degree of disability and payment of disability benefit and it is contingent upon the approval of the competent authority at the Ministry of Defense. A canceled check or certification from the applicant's bank showing the bank account number and details of the branch in which the account is managed must be attached to the claim. The benefit is deposited in the claimant's bank account on the first of each month for the previous month. If you wish to change the place of payment, you must submit a request on the form Notification of revision of personal details, and attach a canceled check or certification from the bank showing the bank account number and details of the branch in which the account is managed. The claim for the benefit must be submitted within one year from the date of the injury. A claim that is not submitted in timely fashion may impair your rights to the benefits.
If you decide to submit a claim for a disability benefit as a result of a hostilities-related injury, please note the following:

A claim is contingent upon approval by the competent authority. Only after the competent authority has determined that the injury complies with the legal definition of a hostilities-related injury, will the National Insurance Institute handle your rights. Please note, various inquiries that will be made by the authority through the National Insurance Institute or through other entities may delay the decision in the matter.
The decision of the competent authority can be appealed. If your claim is not approved by the competent authority, you are entitled to contact the appeals committee and submit a written appeal, with in two months of receipt of the decision. The address for submitting an appeal: the District Court, 1 Weizmann Street, Tel Aviv 64239. The decision of the appeals committee is final.
You are obligated to notify the National Insurance Institute of any change in your personal details, such as change of address, change of family status or change in medical condition. The relevant documents must be attached to the notification (if you have married, attach a photocopy of the marriage certificate; if a child has been born to you, attach the birth certificate, etc.). The notification of change should be submitted to the branch closest to your place of residence. Non- notification of a change in details may impair your rights to the benefit.
The National Insurance Institute deposits the benefit into an account registered in your name alone (holders of a joint account must open a new account in their names only). However, the benefits can be paid to the place on which the claimant is dependent for his livelihood, such as a kibbutz or cooperative moshav.
