Victims of Hostilities - Submitting a claim for a benefit during the period of medical treatment

A claim for a benefit during medical treatment should be submitted on the form Notification of injury during hostilities and claim for a medical treatment benefit. A canceled check or certification from the applicant's bank containing the bank account number and details of the branch in which the account is conducted must be attached to the claim. The claim must be submitted within one year from the date of the injury. A claim that is not submitted in timely fashion may impair your rights to the benefits.

The following documents must be attached to the claim:

        Documents attesting to receipt of medical or psychological treatment as a result of the injury.
        Certification from the local police station. If the injury occurred in Judea and Samaria, certification must be obtained from the IDF.
        Medical certification of incapacity to work, signed by a doctor acting on behalf of a qualified medical service.
        A salaried employee should attach certification of his salary for the three months preceding the injury (each month should be itemized separately, including payments to income tax, national insurance and health insurance).
         A self-employed worker should attach income tax assessments for the current year and the year that preceded his injury, in order to prove the rate at which his income was reduced.
