Risk Pregnancy Benefit - How to submit a claim

A claim for pregnancy bed rest benefit must be submitted to the National Insurance Institute closest to your place of residence on the Bed Rest Pregnancy Benefit Form along with the Pregnancy Bed Rest Medical Authorization Form, signed by an obstetrician-gynecologist.

The claim should be submitted after 30 days of pregnancy bed rest have elapsed.

A salaried employee must ask her employer to certify, on the form, the details of the  months she worked and her salary during the 10 months before she stopped working (if she worked for several employers, she must attach a form from each one of them).

A self-employed woman must indicate on the claims form the National Insurance Institute branch at which she pays her insurance contributions.

A woman who was undergoing vocational training or vocational rehabilitation during the period in which insurance contributions were paid for her must attach to the claim authorization from the Ministry of Health or the Ministry of Industry, Trade and Labor regarding the location and time of the training or rehabilitation.
