| Long-Term Care - Conditions of entitlement |

|
Five conditions determine an insured’s entitlement to a long-term care benefit:
1. He is an Israeli resident who has reached retirement age.
2. His income does not exceed the following:

A monthly pension paid by a foreign country due to the Nazi persecution is not taken into account as income.
3. He is not receiving an attendance allowance for persons with severe disabilities (disabled at work or with a general disability) from the National Insurance Institute, and he is not receiving a benefit for personal care or supervision from the State Treasury (recipients of such a benefit must choose between that benefit and the long-term care benefit available under the Long-Term Care Insurance Law).
4. He is greatly in need of the assistance of another person to perform routine tasks (dressing, bathing, eating, personal hygiene, moving around in the house, etc.) or he is in need of supervision at home for his own safety and the safety of those around him.
5. He lives at home, not in an institution.
A person who is living in a nursing home or in an old-age home (institutions which require licensing of the Ministry of Health or the Ministry of Welfare) are not entitled to benefit, but persons hospitalized in a general hospital are entitled to benefit for the first 14 days of their hospitalization.
After the claim for a long-term care benefit is received at the local NII branch, and the applicant is found to meet all the conditions specified above, he is examined in his home by a professional (a nurse, physiotherapist, etc.) to determine the extent of his dependence on others, or the extent of the supervision that he requires.
According to the results of the nurse’s examination, the National Insurance Institute decides if the insured is entitled to a long-term care benefit and establishes the rate of the benefit to which he is entitled and the period of entitlement.
In cases in which the functional limitation is temporary, the benefit is provided for a limited period of time.
If the insured does not meet all the above 5 conditions, the National Insurance Institute will notify him in writing that his claim for a long-term care benefit was denied, and he is entitled to appeal that decision.
