| Compensation for Polio Victims |

|
The Polio Victims Compensation Law-2007 compensates polio victims who are residents of Israel and who became ill with polio on May 14, 1948 or thereafter.
Under the law, a person recognized as a polio victim who became disabled as a result of the polio is entitled to a lump-sum compensation as well as to a monthly pension or grant, in accordance with his disability percentage or percentage of mobility limitation. The compensation and pension do not affect the other benefits paid to polio victims.

| Lump-sum compensation | |
A lump-sum compensation is paid to the victim, in accordance with his percentage of disability or mobility limitation, determined permanently.

The agreement between the Minister of Finance and representatives of the disabled, specifies that the one-time compensation is to be paid in two equal payments. The first payment is made upon approval of the benefit and the second payment is to be paid a year later. The compensation is paid in addition to the grant or monthly pension, as specified in this section. |

| Monthly pension or grant | |
The pension is paid to a person for whom a medical disability or mobility limitation of 20% or more has been established.
- A person for whom a 100% medical disability or mobility limitation has been established - is paid a full pension of NIS 4,310 (as of 01.01.2012) .
- A person for whom a medical disability or mobility limitation of less than 100% has been established - is paid a pension according to the percentage of his medical disability (or the percentage of his mobility limitation).
Grant (instead of a monthly pension)
A person for whom a medical disability or mobility limitation of less than 20% has been established is paid a grant instead of a monthly pension, which is calculated as follows:
The amount of the proportional monthly pension which would have been paid to him if he were receiving a monthly pension multiplied by 70. |

| How to submit a claim | |
In order to get the lump-sum compensation, as well as the monthly pension or grant, you should submit your claim on the claims form for compensation for polio victims, as of September 1, 2007, in one of the four local NII branches that deal with such claims, according to the following list:
The Ramat Gan branch covers the following branches: Netanya, Ramat Gan, Bnei Brak, Tel Aviv, Jaffa, Bat Yam, Petah Tikvah, Kfar Saba, Herziliya, Ramle, Beit Shemesh.
The Hadera branch covers the following branches: Hadera, Afula, Migdal HaEmek, Tiberias, Safed, Kiryat Shmona, Merar, Nazareth, Upper Nazareth, Shfaram, Karmiel, Krayot, Haifa, Nahariya, Acre.
The Jerusalem branch covers the following branches: Rehovot, Kiryat Gat, Kiryat Malachi, Rishon LeZion, Holon, Jerusalem.
The Beer Sheva branch covers the following branches: Ashdod, Ashkelon, Sderot, Beer Sheva, Dimona, Eilat.
You can submit your form to the local branch nearest to your place of residence, and this branch will transfer the form to the relevant branch that deals with such claims.
