300 - Maternity Allowance and Hospitalization Payment Claim
For insured women who needed hospitalization in connection with giving birth and who are eligible for maternity grant and hospitalization payment.
330 - Risk Pregnancy Benefit Payment Claim
This form is for women who cease work due to the need for precautionary rest as a result of a risk pregnancy, in accordance with medical authorization of a gynecologist. In order to be eligible for payment, the risk pregnancy must be of at least 30 consecutive days and there is no eligibility for payment for risk pregnancy from any other source.
331 - Medical Authorization for the Risk Pregnancy Allowance
The form is for risk pregnancy benefit claims. It is to be filled in by an expert gynecologist and attached to claim form NII/330 while submitting the claim.
355 - Claim for Maternity Allowance or Vacation Pay for Adopting Mother
Claim for maternity allowance for an employee or self-employed woman who took maternity leave or who adopted a child under the age of 10 and who stopped working for the adoption.
368 - Claim to Extend or Split Maternity Grant/Maternity Leave Grant for an Adopting Mother
The form is a claim to extend the maternity leave due to hospitalization of the mother or the newborn for over two weeks during the maternity leave.
360 – Claim for maternity allowance for a father entitled to maternity leave
Claims form for payment of maternity allowance to a father entitled to maternity leave
