| Maintenance (Alimony) - How to submit a claim |

- An identifying document.

Two copies of the court judgment (and of any judgment or divorce agreement on which the court judgment relies), signed and certified by the court or the rabbinic court.

If you are employed - your last three salary slips. A woman who has income from sources other than employment must bring certification of the income.

If you have opened a file in the Collection Office regarding the court judgment that you submitted to the NII, you must submit certification from the Collection Office regarding closure of the file.

For a child to whom a court judgment has awarded maintenance payments and who is not in his mother's custody, a guardianship order or court judgment in which the guardian is the person claiming the maintenance payments for the child.

Authorization from the bank or a canceled check certifying that you are the holder of the account in which you want to receive the maintenance payments.

A printout from your current account at the bank and certification from the bank regarding the amount of your savings and/or deposits.
Please note: you must also provide details of the debtor (his identity card number, address, telephone number and place of employment).
