This questionnaire is designed for the person who informs the National Insurance Institute of his intention to travel abroad, and corresponds to one of the following: an employee sent to work abroad for an Israeli employer, while the employment contract was concluded in Israel, including someone working for a Representation of Israel abroad; those who leave Israel for any other reason and approaches a branch of the National Insurance Institute to announce their departure.
