This form is designed for those who claim an old-age pension from the National Insurance Institute.
You must declare, by means of this form, your income and those of your spouse, for purposes of examination of your entitlement to an old-age pension and increment for a spouse.
The form is designed to be completed by the holder of a stay permit of types A/2 or A/4 as companion of an A/2 permit holder, who requests to be recognized as a resident by the National Insurance Institute (subject to completion of a waiting period).
