How to obtain this service?
A claimant who appeared before a Medical Board or an approved physician is entitled to the reimbursement of incurred expenses in reltion to his appearance at the Board.
Persons suffering from hearing impairments, deafness or muteness, who have been summoned by a Medical Committee, may use this form in order to receive translation/transcription services into sign language during the Committee's hearing.
