The form is designed for the use of those who suffered damage to a medical device (not bodily injury), as a result of a terror act. Such medical devices include prescription glasses and hearing aids used by the claimant before the terror act. The refund is made after the certifying authority recognizes the event as an act of terror, and the amount awarded is according to Health Ministry tariffs.
