How to submit a claim


In order to get the lump-sum compensation, as well as the monthly pension or grant, you should submit your claim on the claims form for compensation for polio victims, and send it to mail or to fax to one of the four local NII branches that deal with such claims, according to the following list:

    • The Holon branch covers the following branches: Holon, Netanya, Ramat Gan, Bnei Brak, Tel Aviv, Jaffa, Bat Yam, Petah Tikvah, Kfar Saba, Herziliya, Ramle, Beit Shemesh.
    • The Hadera branch covers the following branches: Hadera, Afula, Migdal HaEmek, Tiberias, Safed, Kiryat Shmona, Merar, Nazareth, Upper Nazareth, Shfaram, Karmiel, Krayot, Haifa, Nahariya, Acre.
    • The Jerusalem branch covers the following branches: Rehovot, Kiryat Gat, Kiryat Malachi, Rishon LeZion, Jerusalem.
    • The Beer Sheva branch covers the following branches: Ashdod, Ashkelon, Sderot, Beer Sheva, Dimona, Eilat.

You can submit your form to the local branch nearest to your place of residence, the branch will transfer the form to the relevant branch that deals with such claims. 

Documents that should be attached to the claim

  • If you have not been examined in the past for purposes of entitlement to a general disability pension, mobility allowance or attendance allowance, please attach medical documents so that a disability percentage can be determined for you. 
  • If you were not born in Israel, please attach documents testifying to the fact that you became ill in Israel.

Handling the claim

For the purpose of handling the claim, the data on file at the NII for anyone who has submitted general disability pension claim, a mobility allowance or an attendance allowance, will also be examined (the National Insurance Institute has no records from medical committees convened for purposes of income tax). 

The National Insurance Institute will request additional, up-to-date medical documents as necessary. According to the decision by an authorized doctor, you may also be summoned for examination by a medical committee.
