Employers who have an agreement with the National Insurance Institute for the payment of injury allowance directly to employees, under Regulation 22 - are entitled to receive information files about the injury allowances they have paid to their employees.

Those employing 100 employees and above, who wish to join in an agreement with the National Insurance Institute under Regulation 22, shall complete the following forms:
