The questionnaire is intended to be completed by a
self-employed worker contaminated with Novel Coronavirus in his or her
workplace.
The questionnaire must be completed and attached to the
work-injury allowance claim form (BTL/211).
Please note that the questionnaire does not replace the claim form.
