This form is a letter of commitment by the employer who is interested in having an agreement with the National Insurance Institute under Regulation 22, by which he becomes liable to pay a salary to his employees, covered by the agreement, up to the end of the examination of a claim of work injury allowance submitted to the National Insurance Institute.
