This form will allow you to submit a request for medical care benefit or a claim for determination of degree of disability and disability benefit to a hostile action casualty - depending on your medical situation.


Here is an explanation on the benefits:

Medical care benefit for those who did not return to work

The benefit will be paid to a victim who is not working or took a leave from work to receive medical care in relation with the injury, and did not receive any wage or compensation during that period.

  • The benefit will be paid for a temporary period until recovery, based on medical certificates related to the injury.
  • Is eligible for this benefit a victim aged above 14, even if that person has never worked before the injury.
  • From the benefit will be deducted disability payments as hostile action casualty, National Insurance allowances, under the Nazi War Disabled Law, under the Nazi Persecutions Disabled Law, and the pension from mandatory military service or state service.
  • Those who were absent from work, and whose employer paid their wage during the period of leave - shall complete a handwritten request for medical care benefit including the employer's refund request, which will let us examine the possibility of transferring the benefit to the employer. The request can be sent through the document delivery service on the website or to your local branch.

Determining the degree of disability and disability pension

  • The disability pension will be paid to those who kept a physical or psychological injury which limit their capacity.
  • One must attach to the request the medical certificates related to the injury. The request and the documents will be transferred to a medical board which will determine the degree of disability.
