Filling out claims forms according to instructions, will enable to shorten the processing time of the claims and to pay sooner the benefit to employees​.

Employer under Regulation 22

Employers who have an agreement with the National Insurance Institute for the payment of injury allowance directly to employees, under Regulation 22 - are entitled to receive information files about the injury allowances they have paid to their employees.

Those employing 100 employees and above, who wish to join in an agreement with the National Insurance Institute under Regulation 22, shall complete the following forms:

  1. Commitment letter constituting a legal contract between the National Insurance Institute and the employer.
  2. Claim form to join in an agreement under Regulation 22.

The employer must complete above forms, apply his signature and stamp on them, and send them to the Work Injury Department at our Head Office, to the following email address: maasik22@nioi.gov.il
