Requests for reimbursement of medical treatment or medical device related to a recognized work injury must be submitted online using Form BL/234. The form will be available on the website as of January 1, 2026.
To ask for money refund from Clalit, the following documents must be attached:
- A National Insurance confirmation of recognition as victim of a work injury.
- Receipts.
- Whether the material is submitted through a lawyer/third party, a power of attorney from the injured must be presented.
Each receipt must be accompanied with a parallel medical document with the same date (medical visit summary, test results, image, etc.):
- Receipt for the visit to the doctor - medical records of the visit must be attached (medical summary).
- Receipt for the test - the examination prescription + medical report of the test results (interpretation) must be attached.
- Receipt for medication - the doctor's drug prescription or medication recommendation must be attached.
- Receipt for trips to treatment - medical record of the medical treatment provided on the day of the trip.
Please note that as of 1.9.23, the money refund over medical care not provided within the territory of the State of Israel will be made only upon receipts attached to the request.
From this date onwards, money refund will no longer be approved based on hand-written authorizations.
It should be noted that if the refund asked will be made on the injured's name or on a lawyer's name and how (bank cheque or wire transfer to a bank account in Israel only), among the following options:
- Cheque in the injured's name.
- Cheque in the lawyer's name - in this case a power of attorney must be attached.
- Wire transfer in the injured's name - a bank account certificate or a cheque photocopy must be attached, please be aware that this means of payment is only available for Israeli banks.
- Wire transfer in the lawyer's name - a bank account certificate or a cheque photocopy in an Israeli bank only, as well as a power of attorney must be attached.
The refund rate is up to the tariff established by the Health Ministry or up to the actual sum paid by the insured - whichever is lower.
Eligibility for refunds of trips to medical treatments, will be examined as set forth in the agreement and annexed guidelines (a copy is available on the website of the National Insurance under the section "medical care to the work-injured".
