How to submit the claim

The claim for work disability benefits and additional documents can be submitted in the following ways:

• Filling out a claim online and sending it along with the additional documents directly to the branch.
• Filling out a claim by hand and sending it along with the additional documents via our document sending service.
  
• Submitting the claim and documents by mail, fax or in your local branch's service box.

Important to know!

• The claim must be submitted within 12 months of the date of the injury. Filing the claim at a later date may affect the right to the benefit, in whole or in part. It is advisable to submit the claim for the benefit near the end of the period of receiving work injury allowance.
